Cook your bacon in a frying pan as you normally would. Place on paper towels and blot to absorb excess grease. When your pizza is close to being done (about 5 min. to go) add the cooked bacon to the pizza and it shouldn't take long for the bacon to be reheated.

That would be best. You may not have to if the bacon is already room temperature and you cook the pizza at 450' for 13 minutes or more. Once again, it would be safest to pre-cook the bacon and eliminate any chance of getting trichinosis.

That is wise. The bake in the oven is quick to melt the cheese and brown it but not usually enough to cook the bacon. The cooked bacon absorbs the juices from the cheese and stays non crispy.

Yes, cook the bacon before adding it to the pizza. When you cook a pizza you are actually just warming it up and melting the cheese ect. you arent actaully cooking the ingredents. any meat product should be cooked before adding.

I would cook it about half way done to get rid of some of the grease, the bacon will finish cooking in the oven.
