Sincere thank you cards are usually enough. I guess though it would depend on the situation. If you feel it is not enough, give a meaningful gift like a special book, etc. OR take someone out to lunch as a thank you.

It really depends what you are thanking the people for. If it's a holiday, birthday, or wedding present - it should be sufficient, but never leave the card empty inside except for the pre-printed "thank you" and your signed name. I feel (and have heard from others that appreciate the sentiment) that it greatly personalizes the feeling of appreciation if you actually thank them in your own words.
If you are trying to thank someone for something that they did for you - it really is a judgment call. The fact that you sound unsure that the card is sufficient makes it sound like you feel you should show better the gratitude. In that case, think about what the person needs or wants and get them a small (not an extravagant) gift. The gesture will reinforce the fact that you are grateful.

I believe it would depend on the gift. I try to write a little something nice extra in the thank-you cards I write, just so that the person who gave me the gift knows I am genuinely thankful. The thank-you cards you get where it's just like, "Hey, thanks for the lamp. Love Regina." are kind of stiff and insincere. You could always add something like why you loved the gift, what you are doing with the gift now, or even say something about how you miss the person or how great it was to see them. Just make it personal. :)
I think in most cases thank-you cards are enough. How often does someone give you a gift and expects something in return? If you really feel obligated, however, send them flowers, or a gift basket, or take them to dinner or something nice like that. :)
