pat has the right idea. interstitial lung disease is a pathology that attacks the interstitial tissues within the lung. fibrosis is a general term to indicate hardening, thus pulmonary fibrosis is a hardening of the pulmonary tissue, pulmonary vessels, and/or interstitium of the lung. however interstitial lung disease can be a secondary diagnosis with the primary diagnosis being outside the lung. for instance certain arthritic conditions affect interstitial tissue anywhere in the body including the lung; so the primary diagnoisis could be rheumatic arthritis with pulmonary involvment. of course there are many interstitial diseases, ,pulmonary and somatic, thus correct diagnosis can be important to determine direction of treatment. all of it is irreversible.

Pulmonary fibrosis is from scarring of the lung tissues. It means the lung tissues aren't as elastic as normal. Interstitial lung disease is a fairly broad term that covers lots of different causes. Interstitial lung disease can cause scarring of the lung tissue and that results in pulmonary fibrosis.
To be more brief, interstitial lung disease can cause pulmonary fibrosis over time.
There are treatments for lung disease and if effective they reduce the chance or amount of scarring.
