Hi! This is driving me nuts! OK, I usually get sick on vacation - immune sys. shuts down and I spend half of the time buried in tissue box. This past week, no exception - started w/ sore throat xmas eve which has persisted 6 days. in b/t, have had rhinitis, sinus pain 'n congestion, cough. I've tx'd w/ lozenges, Vit C, fluids, boatloads of ibuprofen and a few tyl and/or tyl sinus here 'n there.

OK, so far par for the course. Virus, cold, whatever (btw, i did have flu shot one mo ago...). YESTERDAY, I noted a slight swelling upper hard palate, ant. to vibrating line and 5mm medial to #s 2 &3. today, more swelling, lesion is maybe 3x5mm w/ a smaller 2x2 adjacent. tiss is slightly erythematous, dome-like, non-ulcerated, v tender to palp. no exudate.
my differential so far: salivary? lymphatic? drainage from sinus inf?? it is quite sore and thobs a lot. Any thoughts? (flew Ohio to Bos this AM and spent week driving 'roung OH/MI....?)

OK, i know it's not traumatic, as there's no ulceration. Secondly, I do have a very slight hx of H. labialis - the prodrome tingle and then lesion, tho' that's only occurred 2-3x in my life (mostly in school, i might add...). But again, there's one dome-ish lesion that MIGHT look herpetic, tho' the large one is nothing like....
They are still there going on 36 hrs but they are less painful than yesterday eve. also, i slept for like 14 hrs last night, so that was maybe helpful.

Yes, I considered the parulis/fistula possibility w/ the palatal root....but this is WAY up on the vault - almost adjacent to the midline and not on palatal of the alveolar process.... plus tooth is totally assymptomatic - no hot/cold or percussion sensitivity and no resp to attempting to move indiv. cusps. I have not had a chance to get to the office for a PA however....and I do probably have a good sized comp in there.

thanks for the input kids...keep it coming!

If it was herpetic, it would probably have that "see through" appearance. I doubt it would throb, either. It would hurt, but not throb.

Consider a fistula from the palatal root of #2 or 3. I know, "It can't be!" but maybe it can. My dad had a mystery like this for about a year. We x-rayed it several times, but he had a very shallow palatal vault and it was not easy to visualize that root in the PA film. He was the first patient ever x-rayed in our panoramic machine and guess what. He had an area on the palatal root the size of a garden pea!

Maybe it's a mesiodens breaking through....... The throbbing part gets me thinking in terms of an infection.

Hi there...I was only an assistant (for 12 years)..and this may sound crazy...but I did see something similar a few years back. We ended up using a 7 blade hoping for drainage (even though with exam we didn't think there would be) and it was a popcorn husk.
I myself have probs with sinusitis. 2 & 3 always ache when I am ill. I have venous malformation through our the buccal spaces of my sinus passages. When I am really congested I swear that 2 & 3 are being pushed from the pressure. Maybe you have something simular and it's manifesting it's self on your pallat instead? Have a pan taken and see while the swelling is still visible.
Good luck and take care!

My first thought is herpetic. From location and size. But, with the dome-like structure and the throbing sensation, I'd go to a dds. Even with no exudate, could be an early abcess, but, usually the fistula is on the buccal tissue. I don't know rinse with warm salt water and take some advil.

Hard one to pick. It's probably easier to tell you what it ISN'T.
Salivary, lymphatic, sinus - none of these. Doesn't make any sense anatomically.
Altho it's non-ulcerated, could it be viral? - I'm thinking early stages of palatal herpes set off by your illness. Usually a number of small red lesions up near junction of palatal vault near 1st and 2nd molars. Do you ever get cold sores on your lips?
Nothing else springs to mind. I'd be interested if you ever get a firm diagnosis. Best of luck.

Did you scratch it with something? A lozenge maybe. Could be an irritated saliva gland, lozenges again. Bummer.
