refined sugar is fake sugar made from the sugar cane. it is not good for us. natural sugar which is from fruits do not effect us and is good for us because it is natural. but however if you have diabetes you have to watch your fruit intake, and much be monitored by a diettician. and it can be worked into your diet. good luck

Assuming you are eating the same amount of carbohydrate in each form, the difference is probably the food's glycemic index and your individual reaction to that food.
Glycemic index is a way to rank carbohydrates based on their immediate effect on blood glucose.
Many fruits and some chocolate-y candies are lower GI than white bread (considered a "complex" carbohydrate). (With fruit you are getting fiber, too.)
Do some research on the glycemic index (the Wikipedia page on it is pretty basic and a good place to start) and glycemic load. And, of course, your body is the perfect laboratory if you test your BG regularly.

There are more carbos in a piece of cake than a piece of fruit if you add in flour and other cake ingredients.
To compare , you need the same amount of carbos of both fruit and cake.
The two types of sugar [ carbs ] should effect you in the same way.

complex carbohydrate vs simple carbohydrate. I have never heard one worse than the other, though. Just calculate your daily calories on your diabetic diet and do not worry about it.
