Try a small norstar keysystem, you can probably call a local phone company or telephone contractor and get one install for a few hundred bucks. Phone company will be more expensive, also you will still need to get at least one line from the phone company. This will allow you to recieve incoming calls and place out going calls.

This is kind of open ended. Do you want a searchable database of call records or just a plain jane printout of incoming/outgoing calls? I would call a local vendor and get something like an NEC Elite 48. It is <5k and will do most everything.
