In 1013, London Bridge was burned down by King Ethelred and his Norwegian ally Olaf Haraldsson in a bid to divide the invading forces of the Danish king Svein Haraldsson. The event was recorded in the Saga of Olaf Haraldson, part of the Heimskringla composed around 1225 by Snorri Sturluson. Snorri quotes extracts of a lay by a Norse poet, Ottar Svarte, including the following passage which is strikingly similar to parts of the rhyme:

London Bridge is broken down. --
Gold is won, and bright renown.
Shields resounding,
War-horns sounding,
Hild is shouting in the din!
Arrows singing,
Mail-coats ringing --
Odin makes our Olaf win! [1]
However, the popular version probably originates from 1269, when Henry III granted the tolling right to Queen Eleanor. She is the "fair lady" who notably failed to spend the resulting funds on actually maintaining the structure.

The song is based on an old Norse saga, and was composed around 13th the then queen was given the toll monies by her husband William 3rd, however she was the Coleen Mcloughlin of the day, and spent all the toll money on her flambouyant self.
Needless to say, the bridge fell into serious disrepair. In C1280 the bridge was handed back to the city of London, after the queen could take no more poetic rhetorical sarcasm. That year a terrible winter secured the fate of the bridge, five arches collapsed into the water, and a temporary timber bridge had to be built for that part. London bridge was sold in the 1960's for $2.5m approx, and current stands at Lake Havasu City in Arizona USA

No -london bridge did not fall ever- once they had finally built one out of stone ... the earlier wooden ones were vulnerable to flood and fire.

The first stone bridge lasted 622 years before being replaced by one further north which opened in 1831. The original one was then demolished. The 1831 London Bridge was later transported, stone by stone to Lake Havasu, Arizona in the 1960s, after some American thought he had bought Tower Bridge (this site doesn't mention this, but it IS true!). And the one we have now is the third London Bridge, and that hasn't fallen down either ... yet!
