The causes of appendicitis are not well understood, but it is believed to occur as a result of one or more of these factors: an obstruction within the appendix, the development of an ulceration (an abnormal change in tissue accompanied by the death of cells) within the appendix, and the invasion of bacteria. Under these conditions, bacteria may multiply within the appendix.
The appendix may become swollen and filled with pus (a fluid formed in infected tissue, consisting of while blood cells and cellular debris), and may eventually rupture. Signs of rupture include the presence of symptoms for more than 24 hours, a fever, a high white blood cell count, and a fast heart rate. Very rarely, the inflammation and symptoms of appendicitis may disappear but recur again later.

When the appendix is blocked, it can become swollen and easily infected. The cause of the blockage can be :
by a piece of stool;
by a foreign body that was swallowed; or
the swelling from an infection.

Most cases of appendicitis occur between the ages of six and 20 years. It is uncommon in children under four years, but the rate of perforation is high in this group since young children are unable to tell exactly how they feel and "where it hurts."
Appendicitis occurs in equal numbers in boys and girls.

About four appendectomies (surgical removal of the appendix) are done in every 1000 children under the age of 14.

In most cases, a blockage inside the appendix probably starts a process in which the appendix becomes inflamed and infected. If inflammation continues without treatment, the appendix can rupture. A ruptured appendix spills bacteria-laden intestinal contents into the abdominal cavity, causing peritonitis, which may result in a life-threatening infection. A rupture also may cause an abscess (a pus-filled pocket of infection) to form. In a woman, the ovaries and fallopian tubes may become infected, and the resulting blockage of the fallopian tubes may cause infertility. A ruptured appendix also may allow bacteria to infect the bloodstream - a life-threatening condition called sepsis.

Researchers are not sure what causes appendicitis, but think it may be a blockage between the large intestine and appendix, or a viral or bacterial infection.

The cause of appendicitis relates to blockage of the inside of the appendix, known as the lumen.

The blockage leads to increased pressure, impaired blood flow, and inflammation. If the blockage is not treated, gangrene and rupture (breaking or tearing) of the appendix can result.

Most commonly, feces blocks the inside of the appendix. Also, bacterial or viral infections in the digestive tract can lead to swelling of lymph nodes, which squeeze the appendix and cause obstruction. This swelling of lymph nodes is known as lymphoid hyperplasia. Traumatic injury to the abdomen may lead to appendicitis in a small number of people. Genetics may be a factor in others. For example, appendicitis that runs in families may result from a genetic variant that predisposes a person to obstruction of the appendiceal lumen.

I remember that I was told that wastes moving along the colon enter and exit the vermiform appendix as it also expands and contracts and the chances are that some sharp hard substance in the waste may occasionally cause an injury resulting in inflamation. If the injury is deep and inflammation is prolonged, the infection may lead to appendicitis.

Based on personal experience, my doctors told me there is no cause for it. It just happens. The appendix, for no reason, gets inflamed and must be taken out before it bursts. Upon bursting it can be poisenous if not taken out quickly enough, causing death. I cannot believe it happened to a 2 year old. It was a horrible experience for me at 25.
