I'd just say thank you and tell him to try to be this nice the other 364 days a year.

I'd have to go with the majority here. It may be that the guy doesn't have good social skills. Or maybe someone else made him do it. Either way the point is he did it and he should be thanked. I'm assuming one or both of you weren't there last year - If you were, what happened?

I would want to ignore him but I believe God would want me to turn the other cheek and show him kindness although not deserving of it. Maybe a desktop calendar with nice sayings or funny things so that it might lighten the mood around there.

Say thank you, but don't accept the gift. If he asks why, then tell him why. It happened to me once when I had an awful supervisor. I'm sure he was either Satan himself or a close relative.
