Usually mattresses have a 5-10 year life span. Depending on the mattress and the amount and severity of use.
If springs are showing or poking, if there is more than a 2 inch different between the sides and the center, or if there are body indentations more than 2 inches deep. Or if the box spring is broken or compromised.
If you have to ask, you probably need a new one.
