is it food quality? food preparation?food ingredients? ambience?service? size? what? I have seen small hotels with 4 star rating and large hotels with 2 star ratings.

Widely accepted ratings take all of those things into account.
Size may be considered, but does not necessarily include or exclude an establishment.

Beware agent or travel site specific ratings - for the most part, they rely on other sources rather than actual experience and accept unsolicited reviews (very easy to manipulate in one establishment's favor over another)

Basically:
5 star: Ultimate in luxury. Extraordinary standards of excellence in all areas - rooms, food, amenities, service. First class accommodation. All guest expectations met or exceeded. Many personalized services.

4 star: Excellence. Top quality in all areas. All guest expectations met. Not necessarily luxurious, but shows meticulous attention to every facet of hospitality and guest comfort.

3 star: Stylish, above average in all areas. Typically a lower price than the first two, but not compromising the values of good service, good food and hospitality.

2 star: Good quality (clean, comfortable) accommodation at a reasonable price. Obvious effort made to upgrade and add a few extras to maintain an affordable alternative for the discerning budget traveller.

1 star: Usually basic. Few extras, clean, comfortable, well maintained. Budget friendly.

it has to do with all the things you've just mentioned, these ratings are based on a national guidline in which they are judged based on their performances in all those levels that a hotel runs. a 1 star hotel generally offers little or no service for the guest other than a bed an a room. a five star hotel offers more than just a room for the guest but an ability to explore other options while staying there (like holding meetings, swimming, events).
some large hotels fail to provide such things an in the end can be downgraded to a lower rating whereas a smaller hotel can have less capacity but be rated higher because of their outstanding service.

A four star hotel will indeed cost more than a one star in the same city. The four star hotel will have more services and amenities, including meeting space for conferences. The one star will usually be very basic; not all rooms may have bathrooms in them. I usually look for 2 or three star hotels when I'm traveling in big cities. Today, most offer private baths in the (usually smallish) room and most of the time include television, a phone and occasionally even a mini bar. For a splurge that's often a good bargain, I'll look for a four star hotel in lest touristy areas, especially when it's hot and I want a day with decent air conditioning.

2 lies. According to the history, there shouldn't be anything higher than a 5 star hotel (ratings only go up to 5 star) and very few of those.

Its the overall things that in hotel. They inspect how the service is, how the lanscape looks, the management, rooms, etc...it depends on how the hotel is presented and they well they take care of it.
