For a very long time I used to say "Up is an adverb or a preposition depending on how you use it in a sentence". I was in a high school at that time and it just seemed like nobody in the whole school knew what up was - you heard that question over and over again. Now I am older and I don't hear that phrase nearly as often and so, I am much less annoyed by it - so I usually say "Hey, how's it going?"

It depends...on what's up. I don't give a very detailed answer usually.
It's actually not the question itsself that bothers me, it's the people who ask and don't wait for an answer....because they don't care...they just pretend to care! argh!
