Make sure you stick to a schedule! Every 2-3 hours the puppy must be taken out. Even if it means during the night.
Whenever you go somewhere, you have to either take her with you or have someone go to your house to let her out. She needs to constantly be taken out, so that she will have no mistakes in the house.
Make sure that you take her out at the same times each day too. if you taker her out at 9:00 one morning, the next morning you must take her out at 9:00 again. Making a chart helps with that too.
Another great training method is the bell. Hang a bell by your door, and everytime before you go outside, have her ring he bell. She will eventually know that when she rings the bell, she goes outside. It worked great with my dog!
Another helpful thing would be to buy a dog book. Most books have helpful tips on how to train your dog much easier.

As it sounds like you have had several puppies but never kept them, maybe you shouldn't get a new one.
If you are determined to, you have to be fully committed to the potty training. Crate training works best. Never allow the puppy out of your sight. If it has an accident in the house, it is YOUR fault, not the dogs. Either you haven't taken the puppy outside for awhile, didn't take it out after it ate, etc. You also need to get up in the middle of the night (several times a night for awhile) and take your dog outside. You have to go OUTSIDE with your dog. Standing in the doorway while the puppy goes out into the dark, cold, scary unknown will not work.
Given your past experience, I would read as much as you can and get into a puppy training class ASAP. But the best suggestion - don't get a dog.

Honestly, an untrained dog can be a real pain - a risk to himself, his owner and to other people and animals. Owning an undisciplined dog may result to:

same way you house break any other breed. be sure to look for signs that they have to go - lots of sniffing for the right place, circling in one spot - and be sure to take him out when he does that. also 15-20 minutes after every meal. and at least every 2 hours. you'll soon learn your dogs habits...don't worry! good luck
