If someone is aware that they have an enlarged liver and are down to 4.5 pints of blood, one would have to assume that they are under the care of a doctor.
This is really not the place to be asking questions about something that sounds so serious, hon. Ask the doctor.

Does this person consume a lot of alcohol? Chronic excessive alcohol consumption can cause liver issues, and make bleeding a bit more excessive. I don't think it would actually cause the initial issue, but it could make it much worse.
It might be a GI bleed (somewhere in the intestines). It could be a lot of things, honestly. Make sure this person is under a doctor's care for this issue, because it sounds like he or she definitely needs medical attention.
Hope everything works out...

You have internal bleeding, and quite possible could be getting cirrosis of the the liver. Only a blood count can determine if you need a blood transfusion.

My wife died 2 years ago of liver/kidney failure. She had a liver transplant in 1999.

Go in for Ayurvedic medicines and they do wonders for the liver problems. The south indian name of the herb is "Keera Nalli" ( sounds KEY-RA-NAH-LEE) You take the leaves of this herb and wash them thouroughly and then chew them 3 to 4 times a day. It will be horribly bitter but your liver problem would be solved withing 3 or 4 days. You should avoid salt totally from your food and alcohol is totally prohibitted. The symptoms will start reducing from 3rd day but your diet control and medicines should be continued for 1 month for totall safety. Note first go to a regular doctor and take necessary tests to know the condition of the liver before you do anything I have stated above. Wishing you a speedy recovery.
