It can be. Aside from feeling sick due to abnormal blood sugar levels, you can develop complications, which can be painful.

For example, neuropathy is a diabetic complication in which the nerves are damaged. This can lead to burning, shooting, sharp, and stabbing pains in the hands and feet.

The nerves to the stomach can also be damaged, leading to diabetic gastroparesis. Some people with this disorder suffer severe abdominal pain and nausea.

If an infection or severe wound develops in a diabetic, they may have trouble fighting it due to poor circulation and elevated glucose levels. This can result in gangrene and amputation, both which can be painful.

Also, some people find the daily fingersticks and injections needed to treat the disease (Type 1 always needs multiple injections daily) painful.

I have Diabetes myself, and it, in itself, is not painful, however is your blood sugar levels are not appropriately controlled then you can very quickly become ill with dehydration, heart disease or poor circulation. If untreated, you can also enter a coma (called Hyperglycaemia for high blood sugar).

Depends on which type. I have Diabetes II. It's the type where oral medication and diet will help control it. If you don't control it though it can lead to blindness, bad infections that lead to amputation, and many more problems.
