The simplest answer is the lack of blood flow to the heart muscle (myocardial infarction). Most of the time this is due to a clot forming in one of the little arteries in your heart, in an area where that artery is already partially blocked off by atherosclerotic plaque (hardening of the arteries due to cholesterol). Occasionally it can be due to spasm of the vessel such as in drug induced heart attacks from cocaine or amphetamines.

Heart attack usually refers to myocardial infarction, but lay person might also use it to refer to other heart problems like serious rhythm problems like ventricular fibrillation or ventricular tachycardia.

There are a number of things that can cause a heart attack. What happens in the body is, there is a blockage of the coronary artery somewhere in the hear. Coronary arteries supply the heart muscle with blood flow that carry oxygen and nutrients to these muscles. If the blockage is large enough it occludes the artery and the area of heart muscle supplied by that artery starts to die. When this happens you have a heart attack.

Can be plugged up arteries (high cholesterol or dislipidemia) narrowing of the main coronary artery not due to cholesterol But in any case, insufficient blood supply to heart. Can be tachycardia, extremely fast or slow pulse which requires a pacemaker. Lots and Lots of other things too.

Obesity, Inactivity, smoking (due to its vasoconstrictive effects of the entire body), Diets that are high in cholesterol and fat (e.g. burgers and bacon.) Anxiety, Fear, Stress, High Blood Pressure, Diabetes, Coronary Artery Disease. The list can go on and on because when one of our system fails to do its job, another system will compensate to aid in maintaining homeostasis or balance.
