This looks like it might be an explanation of incident by an insurance company for a patient who's been under hospice care, perhaps under Medicare. This is describing that the hospice patient went to the hospital, whether the ambulance was covered under Medicare and whether physician care was covered once arriving at the hospital.

ER GV is a modifier that means the attending physician (in the Emergency Room) was not employed or paid under agreement by the patient's hospice provider so the modifier code goes on there for Part B Medicaid.

I know there are codes for stating whether the use of an ambulance was deemed medically necessary, but I'm not familiar with those codes. Sorry :(
