TSH and T4. But mostly the TSH level will tell you for Hypothyroidism. I am a type 1 diabetic and I have hypothyroidism and I get my TSH (and t4) levels tested every 12 weeks along with an A1C test.

The definitive test is the TSH. Thyroid stimulating hormone. This blood test will tell your doctor how much of this hormone is present. A too high reading means you are hypothyroid. Too little and you could be hyperthyroid.

Hypothyroidism is easier to treat. Good luck.

Only by a blood test.
There can be some symptoms, but only a blood test can check the levels.
