A systolic murmur is a noise that can be heard through a stethoscope when a doctor is examining the heart. Simply meaning that the blood flow being pumped from the heart is somehow irregular and is causing a sound, possibly due to some deformation or obstruction of the valves in the heart.

A murmur means that we were able to hear the flow of blood with a stethescope. Systole is the time when the blood contracts and pumps blood (when a systolic murmur is heard) while diastole is the time it rests and fills with blood. Most systolic murmurs are quite normal and do not imply heart disease of any kind and most physicians are able to make this distinction most of the time simply by listening without additional tests. If a test is required a cardiac sonogram (echocardiography or echo) is done.

Systolic murmur may be heard due to several reasons, some are due to serious diseases, such as mitral valve regurgitation (that's when blood flows back to the chambers that they aren't supposed to) or some cardiomyopathies, but it has been proven that some systolic murmur can be heard in some cases like fever, or after a hard day's work. You need to identify the source of it to know the severity of the condition, and the following course of action.
