Layers Layers Layers.

Take 3 pants casual and dressy
4 Shirts that can be layered together or worn seperately
I like clean undies so 7 undies if you can wash clothes once
one COMFY bra (or two if you want... I usually take a bikini as #2 cuz you never know when you'll see a hot tub!)
3 Warm socks
one VERY comfortable pair of walking shoes and one pair for "dressy" if you are planning dinners or anything like that... Hikers if you plan on hiking.
Warm vest and rainproof wind breaked gives you a nice coat layer.
BASIC toiletries. Nothing you dn't use literally EVERY day.
I take a plastic bag for dirty clothes. There is nothing grosser than letting the clean get dirty by association with smelly stuff.

Ear plugs if you'll be in hostels. especially in europe walls can be very thin.

Camera. Journal. A good book you don't mind trading when you are done.

A lonely Planet for the country/ city you are going to.

This will get you through. If there is anything you forget Remember that there are stores EVERYWHERE.

Oh! Rainy places might need an umbrella.

If you are moving around alot you REALLY want to go as light as possible. Hard core back packers pride them selves on carrying as little as possble. My first trip was three weeks and I had a FULL big pack and a FULL day pack. My last trip was nine months and I did it with just my day pack.

As you are packing ask yourself these;

Do I actually use this in my real life or is it "just in case"?
How much would it cost to buy this if I ended p needing it?

You can save your back by accepting that you can't prepare for every possible situation, and that if something comes up you can generally buy a Chapstick on any street corner.

Have fun! May the travel bug bite you deeply.
