It is the throbbing of blood through the arteries of the brain. IT IS INCREDIBLY PAINFUL. Generally they hurt from the neck and encompass the entire head. Although some people have "cluster headaches" they are in one side or another of the head. Generally nausea accompanied mine.

A helpful solution or partial solution is a cold pack on the back of your neck to shrink the veins and reduce the throbbing. I also had good success with hot damp towels on my face and the back of my neck. Dampen and heat a hand towel in the microwave for 40 seconds and lay it folded on your face. It feels wonderful, on the back of the neck is good too.

Mine affected my eyes and I could never tolerate light until it was gone. Mine (I learned after my hysterectomy) was from hormones.
They ceased the day I had surgery.

I feel sympathy for anyone that has them.
