They try to "please" others by trying to make themselves look "better". I don't want someone who brags about how small their ribcage is. They're anorexic, bulimic, or something, but they need professional help. And I'm guessing you do get it, just you might be a bit naive. You can easily "get it", but you can't see the mirror the way they do. Its a disorder.

Some people suffer from body dysmorhic disorder. No matter how they look, they always perceive themselves to be fat. Some people who suffer from this also practice binging and purging, or bullemia, or starving themselves, anorexia.
Alternatively, although oddly named, biggism is when bodybuilders believe they are not big enough, and use supplements and illegal drugs to become larger. Alot of these problems have an underlying psychosis, and its best to at least tell your physician if you or a friend suffer from this. More times often than not, a person is just looking for attention, especially the younger teenage population.

It's because they have an eating disorder -- either anorexia or bulimia. The way they view themselves is not the way we see them. It's like looking through a lens(their eye) which distorts everything. They honestly see their body distorted by non-existent fat whereas to us they look like skeletons. If you know someone who has this problem, they need help. Eating disorders (primarily among women but the statistics on men are rising) is a very serious thing.
