You are in man limbo my friend. The sooner you get out of the limbo, the better you will be in the long run. To get out of the man limbo world you must go for broke and talk to her. What ever you do though, you want to be yourself. This means acting like you would act around your friends and family. Do not by any means try to impress her. Women see through this easily. If you use the be yourself approach, you can only succeed, because even if you do not get her number or anything, you have a genuine foundation to build from and learning becomes easier. Life in general is all about turning your stumbling blocks into stepping stones.
Also, if you feel like she is going to say no to you, then do not even try. You cannot set yourself up with a self fulfilling prophecy. Do not expect anything.

It depends. If you've been friends for a while, maybe just try to spend a few hours alone with her and then ask her about it. There really isn't any secret to it. Just be open and honest, and explicit. Really, be explicit. Dropping hints is risky (that goes for any girls reading this too). If you've been friends, you don't have to worry how she feels, because a relationship that started as a friendship was founded on a mutual liking. If you're truly friends, that is you aren't just in it because you 'like' her, then that relationship is safe no matter what.
Now, if this girl is someone new, you'll just have to risk it. Make sure she knows your interested in that way, and maybe just ask her out on a date. She may not feel anything for you immediately (how often does it happen that two people are just mutually attracted to one another without a word being spoken?), but if you can get her to go on one date, and you can impress her, then you can move on from there.
