NOMS is the National Offender Management Service, and incorporates the prison and probation services. They share a common risk assessment tool, OASys, which identifies an offender's risk of reconviction, risk of harm, and areas needing attention.
Probation provides the community arm, delivering community sentences and supervising prisoners who are released early on licence.
MAPPA stands for the Multi Agency Purblic Protection Arrangements. It's a scheme which require probation, the prison service and the police to collaborate to supervise the most dangerous offenders. Other agencies, such as mental health services, social services, housing and education are invited to monthly local meetings of the MAPPP (multi agency public protection panel) to discuss how best to manage sexual and violent offenders to minimise risk.
Sentence planning I'm not so sure about as I believe it's mostly prison based.

It consists of a meeting to discuss how the sentence is progressing, any issues of discipline, and what is going to happen in custody in the future and release prep.
