This is probably just a form asking for vet references and a basic background check to make sure you don't have any (animal) skeletons in your closet. It probably also asks whether or not you plan to crate your dog, keep it outside, feed quality food and not abuse it.

Any good and decent adoption agency will ask you to fill out a form or application. If you are serious and want this dog for a pet, just be honest with your answers and why you want the dog. They just want to be sure it is going to a decent loving home. They are being safe. They will probably ask for references. Give them names and phone numbers of people who have known you for a long time and know how much you want the dog. Good references are friends that have pets and know how you treat them.
Anyone can buy from a pet store..it takes a special person to take in a dog from a shelter..some of them are horribly abused and need special love and care. Make sure you want and can keep this dog, Good luck

Just be honest and answer the questions as completely as you can. The rescues use the form to choose the best fit for each individual boxer because they want the dog to have the best home possible. Some boxers need more attention/exercise/training than others; some need less. Let the rescue help you find the right dog for you. The only way they can do that is with honest, complete information about you and your lifestyle. A good match will mean that you and your dog will be as happy together as possible. Good luck!

It's not uncommon for animal adoption agencies to demand that potential buyers/adopters fill out paperwork. This gives the agency not only contact information, but also helps them to screen potential adopters to make sure they're a good match for the dog. Often the paperwork also includes contract-like agreements between the agency and the adopter to specify who will be responsible for what as regards the dog.
At our agency we screen all adopters for compatibility with the dogs, we also pose hypothetical scenariio questions to see if the adopters have basic common sense when it comes to having a dog in their home; we do home checks (often ask for photos of the home / yard for our files), and do interviews... all before the adopter is "approved".

It's perfectly normal to have a form filled out to adopt a dog, they do it for the dogs welfare to make sure it will be a good fit for both of you, nothing to worry about

If you cant read or write take someone with you who can. Make sure you take a pictured ID and proof of you living arrangement. Like Landlord permission or deed. You really need to add more details..........OK then it will look better for you if you take a vets recommendation letter also. Enroll in a training class too.

Well, fill it out honestly and that is all you can do. They are usually good at spotting "fake" answers, so don't just tell them what you think they want to hear!!
