I had one installed a little over three years ago and the unit feels almost non existent.
There will be some tenderness around the implant, mine lasted for about six months till scar tissue built up around the unit.
The operation is a very simple one, I was awake the whole time till they tested the unit, then they put me under, what actually happens is that they actually stop your heart and wait till the unit discharges and re starts your heart, if the unit doesn't do the restart they will shock you with the paddles to normal rhythm and reposition the leads an do it till the unit actually jolts your heart back into normal rythum.
Sounds like its a little extreme, but not to worry you'll be in good hands.

If you would like further information about your procedure please feel free to E-mail me at Iggy465@yahoo.com - I am on my third unit because of recalls because of defective batteries and unexpected discharges when one was not needed.

I had one put in march 2005, It is really not too bad, I can feel it now and then just regulating my heart(only if i sit just right). Other than that, it gets a bit tender now and then, and i think they must have cut thru some nerves or something. Cause once in a while it will ache a bit. The biggest problem is holding your arm perfectly still in the sling for 6 weeks while the leads heal into your heart tissues. That sucked!! Good luck, but its really not bad, even the surgery isnt too bad!
