Can pinworm be cleared through good sanitation and without medicine?

Pinworm is often reffered to as threadworm as well

yes im sure i've had them for a while now and no medication has worked too embarassed to get it too because i feel im a bit old to have them

Well, if you are sure you have pinworm infection it can only go away with medication, unfortunately. It is usually pretty easy to treat with medications and there is no way to get rid of it with sanitation only. Did you have a stool or "tape" test done?
