We're not entirely sure, but the fact that so many diffferent peoples had legends about women or horses or other mammals becoming pregnant from winds or standing in moonlight or drinking a particular magic potion or other things is a clear indication that we haven't always known.

Since evolution didn't happen. When God created Adam and Eve, their minds were perfect and they would have been very capable of learning, so I'm sure that they made the correlation very soon that sex produced children. He did tell them be fruitful and multiply, so it makes sense that they knew, God may have implanted the knowledge when He made them. I imagine when they realized how good it was, they had no problem fulfilling the command to be fruitful.
