If you're just having a vaginal exam and they do an STD test from there, they are gathering cultures of tissue from your vagina and cervix. The only things that can determine from this are whether or not you have chlamydia, gonnorrhea, or HPV. Most will also check for yeast infections and two variants of bacterial infections. These are not considered an STD, but they can on rare ocassions be passed to a woman from a man who is having sex with a woman who has either infection (whether it's another woman or if you just got treated for one but were still having sex with him before it cleared up).
The doctor will do a visual test to make sure you have no obvious bumps and such that may be herpes or warts. If he does find those, he'll do a skin culture to collect tissue and check it.
Syphilis and HIV tests are done with a blood test.

They lay you on a couch and stick this giant probe with a human eye on the end of it between your legs. After you are done screaming they tell you if you have herpes, aids or gonorrhea. Then you have to tell a strange doctor about all of the sex partners you have had in your entire life. All of the greasy details. If you are under 18 your parents have to be in the room the whole while.

It depends.....most test for an RPR (syphillis), gonorrhea, chlamydia, HIV and usually a hepatits panel is done....but again...depends on what the doc orders. All of the above is a simple blood test/vaginal/penile swab.
