Your reservation form has a portion in the lower half asking for onward reservation required. Fill in the concerned train name and number and particulars. That's all you need to do . The rail Department does the rest for you. Nowadays they charge for the service, but only moderately.

The booking form has provision for onward booking as well! You can book rail tkts 'on-line', too!
