The heater & engine is full of coolant you took it in three times this year? or in the past? A Altima is no Lexus... I most times find the heater blend door cable broken off of the heater controls have some one turn the center knob smoothly back and forth while you look at the heater blend door on the left side of the heater case heater is on the transmission tunnel and what you are looking for is the cable and curved gear that operates a white plastic gear covered in red grease you can push the cable and rack gear to full hot and see if it heats up any better. I have seen the cable bent at the a/c control head and a/c control head broken where the cable hooks to it. The other thing Altimas do is get air trapped in the heater and you will hear rushing water sound when you rev it up. Burping the cooling system should be left to some one with a vacuum type radatior refill system.

Could be a design flaw, or perhaps the thermostat is opening too early, meaning the thermostat is faulty, or a thermostat that is rated at a lower tempurature has been installed. Perhaps your vehicle has a filter that is clogged, or a blockage in the the heater core sytem.

After your vehicle's water temperature gauge has reached the normal operating temp, how long would it take to heat the interior?

maybe you should call Nissan Corporate and complain. I know it sounds simplistic, but it might be the last resort. If its still under warranty, that would be a good thing. good luck.
