Nobody is required to have legal counsel. A person is allowed by law to act in their own defense. Remember the old saying..."A man who is his own lawyer has a fool for a client."
- Hunt
You are certainly permitted to dismiss ANY legal counsel and act as counsel in your own defense, but I highly recommend nobody do this.

If a judge ordered a person to see the public defender and if the person failed to do what a judge ordered, he/she can be held in contempt of court and thus be sentenced to jail for such.

I agree with the first answer. But the point of it is to get your case moving along. So, if you are not going to see the Public Defender, you need to either hire your own lawyer and have the lawyer show up at your next court date, or decide to represent your self (in pro per).

Best advice you will ever be given: Do what a judge says.
If a judge told me to do jumping jacks I would ask how many.

No, no law says that you have to accept counsel. You can defend your self , if you wish.

Yes.
If the court has appointed a defense attorney for you, and you do not keep your appointment, you are in contempt.
