Go to a family court mediator. They usually have "ready made" documents that you could use for temporary guardianship. it costs nothing for their services. Mediators are also good for other family matters, or just to even get advice. I hope this helps.

Probably should call your local family courts...they'd be able to tell you the proper forms to fill out and what-not. Also, see if you can get a free consultation with a lawyer...sometimes they'll be able to guide you in the right direction.
