You can get it at your local health department. I don't know if it is different for Arizona, but I think you can just walk in and tell them you need to get your food handlers permit and they will let you take the test on the spot. It costs like $15.00 (may change due to location). They give you a booklet to study for the test, the test is I think 50 questions. You can choose to go home and study, or study there and then take the test. The test is really easy though, most of it is common sense, I would recommend studying there at the health department for a few minutes then taking the test. I'm not totally sure, but if I remember correctly you can only miss 2 questions.

I got mine in high school (Foods class). try asking the school's around your area. it would probably be best if you asked a food/home ec. teacher if he or she can license you. sometimes the front office doesn't know much about the courses taught at their schools. if they can't help you, then ask someone at a restaurant.
