I use a dremel (grinder) to make a small indent in the tile, then use any standard carbide tipped drill used for drilling cement. Drill slowly.

You use a ceramic drill bit. Use 2 pieces of masking tape crossed over the spot to be drilled + and go slowly. Let the bit do all the work and stop often to make sure the bit doesn't overheat and crack the ceramic.

Even doing all this, I have cracked more than I wished, but less than just drilling away.

The only thing you can use to drill through any type of ceramic or glass is a Diamond Tip Drill Bit. These specialty drill bits are available at Lowes, Home Depot, Michaels, or most specialty craft stores. Depending on the size drill bit you need the cost ranges on average from $4.00 to $16.00.

With great care!! (Sorry, I couldn't help myself!). Seriously, there should be a drill bit available at any good hardware store which is designed for drilling through glass and ceramics. I'm in South Africa, and I was able to get one - I wanted some holes in a glass bottle.

If this is a one time project, you might consider calling a glass shop. They have all the equipment and it may be cheaper than buying the tools.

use sharp drill and masking tape on both sides to keep from chipping
