A self-employed individual who is also an employer will file forms W-2 for his employees.

The form 1040 will be used by a self-employed person to report income - it is the personal tax return.

Form 1065 is for partnerships and partnerships are a merger of two or more self-employed people (may also be one person and a corporation but lets not make this any more complicated than it needs to be).

Form 1040ES is used to compute estimated payments, although it isn't actually sent to the IRS.

So the above doesn't really give a clear answer, if you think of it in those terms. Personally, I think the 1040 is the best answer. The others are a bit of a stretch in terms of the question posed.

A self-employed individual will file a Schedule C with his Form 1040. He would also most likely be making estimated payments each quarter of the year to the IRS via 1040 ES tax payment vouchers. Additionally, if he had employees (he or she does not count as an employee of themselves), he would be required to prepare Form W-2's for his employees after the year ended.
