It's a slang term originating in the Boston area in the 1950's, coming out of the then very thin- on-the-ground drug culture. It started out as an alternative to "cheesing" it, another inexplicable application of slang to getting out of an area quickly.

Someone said it "off the top of their head" one day. Someone heard it and used it, it caught on and other people started saying it. It grew from there.

Just a guess, but could have been what happened.

peoples names were written in a book ,when they made a resarvation in a restaurant or before the computor ,with travel agencies or hotels (the registar)
so when your name went into the book you were booked
and the name stuck and was taken into the computor age
today it should realy be called -entered into memory

how did "the sh*t" become a compliment? same reason...just a colloquialism that took off.
