Yes - the individual gets his/her own number that can be used with multiple practices. There is also a group number that is used.

Yes. In the past - each location would be issued a different number. Medicare 1st instituted the Universal Provider Identification Number, and more recently the National Provider Number. All physicians must have both - and no physician may have more than 1 number. Thus - the single UPIN and NPI numbers must be used by the physician at all locations.
