The Islets of Langerhans in the pancreas. In normal production, the pancreas supplies enough to break the sugar down into a simple form that the body can use, and is able to pass thru the semipermiable membrane around the cells.

If there is not enough insulin, the sugar is not usable at the cell level and the patient has a high blood sugar.
