A few senarios that jump to mind are -
a) Vitamin D intoxication: Remember Vit. D is a fat soluble vit. and can accumulate in the body.

b) Granulomatous diseases like sarcoidosis..reason is that they cause an artificial state of vit.D excess due to synthesis of 1,25D3 by the macrophages in the granulomas.

c) Blood malignancies- Myeloma would be the classical one but leukemia and lymphomas too.
hypercalcemia is due to bone mets, synthesis of PTH like peptides while the high cell turnover (especially when on chemotherapy- tumor lysis) will cause hyperphospatemia.

d) Solid tumors can also cause the above mentioned senarios for pretty much the same reasons.

Hypercalcemia is a higher than normal level of calcium in the blood. The most common cause is an overactive parathyroid gland (hyperparathyroidism). The parathyroid glands, which are located below the thyroid gland, regulate calcium in your body.

Other causes of hypercalcemia include:

Certain medications, such as lithium or thiazide diuretics
Other causes of hypercalcemia include:
Certain cancers, including breast, lung and certain blood cancers
Sarcoidosis, an inflammatory disorder
Excessive intake of calcium or vitamin D supplements
Familial hypocalciuric hypercalcemia, a genetic disorder
Dialysis for chronic kidney failure
Adrenal gland failure
Overactive thyroid (hyperthyroidism)

Hyperphosphatemia occurs when the phosphorus load (from GI absorption, exogenous administration, or cellular release) exceeds renal excretion and tissue uptake. Conditions that are associated with hyperphosphatemia are listed below:

Cancer
Leukemia
Lymphoma

Bone tumors
Other cancers

Chemotherapy treatment
Endocrinopathies
Trauma
Burns or heat-related illnesses
Prolonged immobilization
Metabolic or hematologic disorders including genetic predisposition
Medications
Oral phosphate binders

Potassium phosphate
Antacid use

Bisphosphonate therapy
Use of laxatives (oral/rectal) and enemas
Use of nutritional supplements or hyperalimentation
Ischemic bowel (possible phosphorus elevations)
Phosphate and calcium together would cause precipitation of calcium crystals. As these two electrolytes are essential antidotes for each other, the only condition (other than renal disease) that affects both calcium and phosphate is parathyroid hormone and bone tumors (primary and metastatic) Usually these conditions present with hypercalcemia and HYPOphosphatemia. I did a literature search and the only thing I could find with these two conditions happening simulanteously, was a very rare endocrine disorder. Good luck.

I had a cat die of renal failure at 16. My vet said that an all dry diet contributes to the probability of this happening. Cats don't drink enough anyway, and tend to run dehydrated if they only eat dry food. Don't feel guilty, it is something new that is just coming to light. If you caught it in time, you may have to give her subcutaneous fluid injections for the rest of her life.
