Nearly 60,000 U.S. heart transplants have been done since Dr. Norman Shumway's first on Jan. 6, 1968, at Stanford University. I cannot find the patient's name - perhaps because s/he only lived for 14 days.
Barnard performed the world's first heart transplant, but that was in South Africa, not in the US.

SAN JOSE – Norman E. Shumway, the pioneering Stanford surgeon whose skill and dogged dedication made heart transplant surgery safer and more successful, died on February 11, 2006 of cancer at age 83.
Shumway and his Stanford surgical team performed the first human heart transplant in the United States in 1968 on a 54-year-old East Palo Alto steelworker named Mike Kasperak, who lived for 14 days after the operation.
