There is a legend that I have seen recounted in at least two different Mexican cookbooks, that claims the famous dish "mole poblano" was created by accident. Supposedly in the 16th century or thereabouts, nuns in Mexico were preparing an elaborate meal for a very important visitor (the viceroy, an archbishop or someone). The door of the kitchen was open. A gust of wind came up, and blew all the different spices into a single pot.... Or that's the legend, which is in one of Diana Kennedy's cookbooks and also in (I think) "Cocina de la Familia," also an excellent cookbook by Marilyn Tausend and her co-author whose last name I always forget although he was chef at a wonderful restaurant, Fonda San Miguel in Austin, TX.

In all likelihood, cooked meat was created by accident when clumsy Grog dropped the hunk of wild pig he was chewing on into the fire they were burning to keep the sabre-toothed tigers away. When the burned meat smelled so good, he started doing it deliberately, and soon the neighbors smelled it and came over with a pot of that fruit juice they left out too long that had been fermenting from wild yeasts...

Salt Water Taffy. There was a candy stand facing the beach on the boardwalk in Atlantic City, NJ. One night after the stand had closed, a high tide washed ashore and dampened his stock. He specialized in taffy and marketed what he thought to be a "wash" (pardon the pun) into a huge seller.

Nachos! Rumor has it that a group of travelers happened upon a restaurant late one night and were looking for grub. The kitchen was closed at the local restaurant but the cook, a man named, Nacho, took what he had left over-some tortillas, guacamole, salsa etc., threw it all together and served it to the weary travelers. They loved it and called the concoction, "Nachos"!

Blackend snapper, or anything else for that matter, it was over cooked by accident but served anyway with the story that it was a new creation. The fool bought it.

The Chocolate Chip Cookie, a woman was making butter cookies with chocalate and didn't melt the chocolate down first. And then chocolate cookies were born!

potato chips were invented by a chef who intentionally cut his potatos extremely thin to aggravate a customer who complained that the potatos were cut too thick.
apparently they tasted good and the idea stuck.
