cardio = heart
vascular = blood vessels

It is a disease of the blood vessels that serve the heat muscle tissue. Blocking these vessels means that the heart muscle tissue doesn't get enough oxygen causing pain (angina) or even dearth of some of the heart tissue as might occur in a heart attack. If enough heart tissue dies, the heart fails to function, leading to death.

The cardiovascular system is the vessels (or Arteries, Arterial Veins, Veins and capillaries system) that carries blood, nutrients and gases around the body which is pumped by the heart.
Cardio = Heart muscle
Vascular = Veins, arteries etc etc.

Cardiovascular Disease therefore is a disease, of this blood transport system, ie furring up (clogging) of the arteries, veins etc due to a combination of high fat un-healthy diet, smoking, lack of exercise etc which all contribute over time to the demise and workings of this system, which will also lead to angina, heart attacks, poor circulation etc.

Just as WildOtter has said, it means any disease that either affects your heart or your blood vessels or both. If the disease is an obstructive type than the lungs usually get involved.

It's basically a group of diseases that affect the heart, major arteries and pulmonary/respiratory system including the lungs.
Coronary heart disease, heart attacks, atheroma (hardening and furring up of arteries), emphysema, chronic bronchitis, cystic fibrosis etc are all cardiovascular illnesses.

Put simply it's caused by the arteries furring up, that's when it's called ischaemic heart disease which causes restricted blood flow which as you will know can kill you if untreated.
It is treatable today but 15 years ago it was a death sentence.
