One of the definitions of ALS is muscle wasting and weakness without exertion. I imagine soreness is part of it, too. You need to ask the patient's neurologist about any symptoms you don't understand.

It is a good sign the first few times you work out or after a particularly tough workout. If you are consistently sore after moderate workouts then you may have a problem. When you first start training you will be sore and that is a sign that you are getting stronger.
