Unprotected anal sex is one of the main causes of HIV transmission. The virus moves into the bloodstream through small tears in the skin of the anus, which is very easily torn.
Having unprotected anal intercourse with a HIV infected person is not a guarantee that you will contract the virus but you should use a condom and water based lubricant every single time (that goes for vaginal intercourse too).
Better safe than sorry.

It is entirely possible to transmit HIV through breastmilk, so if this woman has got HIV then so could your friend and the woman's baby. All three of them should be tested for the virus (your friend especially as he may have contracted it via sexual intercourse). But what makes you think that this woman may have HIV? Is it a random guess? Are you paranoid? Did she say she might?

The HIV viirus is transmitted through exchange of bodily fluids (including semen, vaginal fluids, blood, etc.). During any form of sexual contact this can occur; not just anal sex. So if a person is infected and you have sex with them you are putting yourself at risk. However, there are many ways to reduce the risk significantly and anyone (and I mean anyone) who is having casual unprotected sex even with someone they think is HIV- is nuts!
