Blood tests for the cardiac enzyme creatine phosphokinase MB (CKMB) appear in blood test results within four to six hours after a heart attack and peak within 12 to 24 hours. After two or three days, cardiac enzymes can no longer be detected by blood tests, so these tests are only useful to detect recent heart attacks.

An EKG or electrocardiogram should show proof of a heart attack immediately. As far as blood testing I would imagine that free troponin probably starts showing up in the blood within an hour. In a true myocardial infarction (heart attack) the muscle dies from lack of blood flow. As the muscle tissue breaks down it releases troponin into the blood stream and this can be measured to approximate how much muscle tissue was damaged.
