Are you sure? My son has a medical alert bracelet and we ordered it from Walgreen's. If you don't see the form ask the pharmacist because I have seen them in every drugstore that I have been in. Good luck

Most stores have phamplets that you can order. However at a WAl-Mart they do sell them off the shelves without any words on it. I ordered mine through a company. Do you have a shop-n-save supermarket that has a pharmacy or what about a Wal-Mart? If you have either one of these stores in your area please go to the pharmacy department there you should see some phamplets that orders those bracelets. I have one of those phamplets and below I have listed the company's name, address, phone number and web site for you to contact in case you do not have any of those stores in your area. rite aid. check by/ with the pharmacy...they may have a order for or may be able to lead you in the right direction. hospital supply store maybe where they sell gram pa diapers in bulk. have ya checked around on line??? good luck
