That is actually one of the more serious side effects of methadone and this site says to call your dr ASAP if you get that side effect - same with the weight gain - that is from the water and swelling. It is very easy to gain that much "water weight" over night with such excessive swelling - this person needs to call their doctor immediately. It is from the methadone most likely - even if it has not happened before, now - his body is rejecting it and having a bad reaction to it. Of course, only a dr could say for sure what the cause of this is but it is listed right under dangerous side effects so I suspect this is what the cause is.
Of course, there could be other problems with the liver - if this person was using illegal drugs before methadone, that could cause a problem with the liver and it would not filter toxins or drugs as it should - and they may be abusing methadone too as it is also very addictive and dangerous. This person really needs to start to take better care of themselves - very sad.. this drug can make them depressed too. It is nice that you care.
They should have the liver function tested - bilrubin and the other liver functions - there are 4 tests I believe so that is what the dr is trying to get tested. And it is very important that they get this done. Drugs are obviously taking over his/her life and so much so that they don't even care about their physical health anymore - methadone is considered safer than heroin but it is very widely abused and take for much much longer than necessary - this could kill him - the methadone that is - more and more people are dying.
What are the possible side effects of methadone?
• Get emergency medical help if you have any of these signs of an allergic reaction: hives; difficulty breathing; swelling of your face, lips, tongue, or throat.
• Call your doctor at once if you have any of these serious side effects:
· difficulty breathing;
· hallucinations or confusion;
· swelling of your ankles or feet;
· urinating less than usual;
· chest pain; or
· feeling light-headed, fainting.

He is retaining water and the kidneys have started to fail. The same thing happened to my uncle and he is a diabetic. He gained 100 pounds from water retention and was a hefty 350. After getting pumped full of antibiotics, he lost 150 pounds. It was unbelieveable. He looked like a new man after that. I suspect maybe your friend has diabetic tendencies.

there may be some lymphatic thing going on there. they really need those blood test to narrow down to an ending result/disorder.
anytime a person gains that amount of weight in that amount of time, there is a huge cause for concern. Are they having chest pains yet? They should be. Get the test done ASAP.

it may be edema since edema is caused by excessive liquid in your body going into the tissues causing it to swell...to check it out, you can press the swollen area and if it creates a pit, then it is definitely edema...a lot of factors can cause edema but most common is electrolyte imbalance where sodium may be too high in your body and sodium causes water retention therefore causing excess water in tissues making the extremities swell...avoid eating salt and try if that works...because your kidneys may have impaired functioning
