I need an acceptable generalized FMLA form to request emergency leave from a small business I work for here in Arizona. I cannot find this form on the Dept of Labor website nor anywhere on the websites for the Arizona government.

Ask your supervisior where you can get an FMLA form. Most larger compainies have them in the Hiring (Human Resource) office. If your supervisor can not help you, then call your primary doctor and inquire about a form. I heard that our company has a more condensed version, but the doctors office has a nother suitable form.

I took FLMA for 6 weeks when I had surgery last year. I was paid the whole time from my Extended illness bank fund.

You should be able to get one through your HR department, if there is one.

Otherwise, check with the doctor's office.

I recently took FMLA leave from a larger institution in Maricopa County, and the HR dept sent me the forms, and I took them to the medical center where my father was being treated. The medical center filled out all the forms and sent them back to HR...all I had to do was sign in a couple of places, and that was it.

HUMAN RESOURCE DEPARTMENT ON YOUR JOB SHOULD HAVE THAT FORM.
