You get to pick your own confirmation name. I think you get it on a Confirmation certificate, but no, your birth certificate will not be altered. Only the catholic church is interested in this name, not the state (unless you suddenly start using it). I can't even remember what mine was.

Do you know what you are doing? are you just following these steps because that is what you do at that age? It is not the church or the priest nor is it Baptism that saves you it is only the Lord Jesus Christ. When you get to the point you know you are a sinner and you need a saviour then you ask the Lord to forgive you and you repent of your sins, ask Him into your heart and make Him the Lord of your life. It is then you become saved and nothing can ever separate your from His love or take you from the Father's hand. Upon doing so you should then desire to become baptized as a first step of obedience to the Lord. No where in the scriptures is there any record of infants being baptized. Total immersion is how it was done in the scriptures. Hope this helps you think about where you stand. You see if it is in the scriptures then you know it is of God all else is of man.

You can pick any Saint's name or you can let your sponsors do it.
If you want the name on your birth certificate you will need to do a legal name change.
I picked two Saints actually, the Virgin Mary and St. Michael the Archangel. Thus my confirmed name is Mary Michael.

if you are other denomination happens
no change to your birth certificate happens either
